We must have official
school records in order to process your application. The following criteria
must be met in order for the records to be considered official. The University
of Utah Admissions Office determines what is considered “official.” Please
follow these guidelines.
1.
School
records must be issued by your school. Records typed or translated by the
student are not acceptable under any circumstances.
2.
The name
and location of your school must be printed on the school record. If your
school does not supply such official forms, school records must be sent with an
accompanying letter on the school’s official letterhead stating that the
enclosed school records are official. These must be sent in an official school
envelope to the University of Utah, Admissions
Office, 201 South 1460 East, Room 250 S, Salt Lake City, Utah, 84112-9057,
U.S.A

Besides the
name and location of the school, the school records must include the student’s name,
birth date, identification number (if any), major area of study, and entering
and leaving dates. Classes for each semester or year should appear, as well as
hours and marks or grades. A grading system should be provided.
4.
If not
printed in English, the school must show the original seal and a signature of
an authorized school official in the native language and in English, preferably
with an embossed seal.
5.
If not
printed in English, the school records must be sent with an English
translation. Both documents must be official.
6.
If erasures
or changes of any kind are made on the school record, it will be considered
unofficial.
There will be no exceptions to the
university’s policy concerning official school records. All applications
received without official records or the required application fee will be filed
with no further action taken.
